Root cause analysis and corrective action
A two-day course that teaches people how to investigate incidents properly, find the real causes and write corrective actions that hold.
The course incorporates ICAM (Incident Cause Analysis Method) and HOP (Human and Organisational Performance). Participants learn to gather evidence, build a timeline, analyse contributing factors across people, environment, equipment, procedures and organisation, and design corrective actions that address the system, with the people who do the work at the centre of the analysis.
What participants learn
- How to plan and run an investigation from notification to close-out.
- Evidence gathering and interviewing, including how to talk with workers after an event.
- ICAM analysis: absent or failed defences, individual and team actions, task and environmental conditions, and organisational factors.
- HOP principles: error is normal, blame fixes nothing, context drives behaviour, learning and improving is vital, and how leaders respond matters.
- Writing findings, root causes and corrective actions that stand up to scrutiny.
- Applying the method to a realistic case study built around the participants' own sector.

Practical training for people who audit their own organisation's ISO 9001 quality management system, health and safety system, or both.
Internal auditor training is delivered on behalf of NZOQ, the New Zealand Organisation for Quality, and in-house for organisations that want the training run on their own system.
The course follows the ISO 19011 approach to auditing management systems and applies it to the participants' own system, whether that is an ISO 9001 quality management system, a health and safety system, or both. It is delivered by a Lead Auditor and Certified Quality Manager who has run internal audit programmes and been on the receiving end of external ones.
What participants learn
- The purpose of internal audit and where it fits in the management system.
- Auditing against the clauses of ISO 9001, from context of the organisation and leadership through to performance evaluation and improvement.
- Planning an audit: scope, criteria, sampling and checklists.
- Conducting the audit: interviewing, observing, reviewing records and gathering objective evidence.
- Writing findings that are clear, evidenced and useful, and grading them consistently.
- Reporting to management and following corrective actions through to close-out.
- Practice audits using the participants' own procedures and records.
